  • Central East LHIN Integrated Health Service Plan Survey

    Background

    The Central East Local Health Integration Network (Central East LHIN) is developing its fourth Integrated Health Service Plan (IHSP), which sets out shared goals for the organizations and service providers that make up the local health care system including hospitals, community support service agencies, community health centres, the community care access centre, community mental health and addiction agencies and long-term care homes.

    As the Central East LHIN develops IHSP 4, they are inviting patients, clients, consumers, residents, caregivers, and health service providers to complete a brief survey.

    In IHSP 4 (2016-2019), the focus will continue to be putting people and patients first – improving their health care experience and their health outcomes – as outlined in Ontario’s Patients First: Action Plan for Health Care.

    This will mean continuing to improve access, better connecting services, and providing people with the education and information they need to make the right decisions about their health. In the Central East LHIN, this will mean helping people to live healthier at home.

    Survey for patients, clients, consumers, residents, and caregivers

    The Central East LHIN wants to hear from patients, clients, consumers, residents, and caregivers about their experiences with the health care system, their goals for living a healthier life in their homes and communities, and how that can be achieved.

    Survey for front-line staff, managers, senior administrators, volunteers/board members

    They also want to hear from health care service providers about the challenges they face in delivering co-ordinated and comprehensive care to their patients, clients, consumers, residents, and their families/caregivers and the opportunities they see in better supporting them to live healthier lives in their homes and their communities and how that can be achieved.

    Survey feedback is being collected up to and including Friday, September 18. All feedback will be reviewed and considered before the IHSP is submitted to the Central East LHIN Board at the end of October.
