Acute Care for the Elderly Program

The Acute care for the Elderly (ACE) Program is an inpatient acute care program. The TSH care team works with these patients, enhancing and maintaining their functional and cognitive capacity, so they are well enough to return to the community. Our efforts to ensure the wellness of our elderly patients helps to minimize the need to transfer patients to long term care facilities.

Patients may be referred to the program by any member of the care team or by the Emergency Department, The Rapid Admissions Unit and the in-patient units.  Medical supervision works closely with the team including nursing, physiotherapy, occupational therapy, social worker, pharmacy, nutrition, and speech language pathology as required.

Patients greater than 65 years of age with varied medical challenges will be assessed for participation in the program. Each patient is admitted into the program for a maximum of ten days with goals and objectives for success.
Our team works closely with each patient every day to discuss the benefits of meeting his/her goals and any challenges that may affect discharge planning. 

Together with the patient’s family, the care team will meet to communicate the patient’s progress as requested or required in order to create a seamless transition in to the community.
