SURGICAL PATIENTS
Pre-Admission Program
The Pre-Admission Program prepares you for your operation.  A Pre-Admission clerk will call you two to four weeks before your operation to book an appointment that may include individualized teaching and lab tests, and/or consulations that need to be done before your operation takes place.

The location of Pre-Admission Services is in the basement of the Medical Mall building that is attached to the main hospital (take the elevator to "Lab" in the basement). The entrance to this building is directly across from the Visitor’s parking garage. The phone number for Pre-Admission Services is 416.431.8176.

When you receive your Pre-Admission Package please make sure that you read it completely and fill out all forms before arriving at Pre-Admission Services.

You will need to bring to your Pre-Admission Appointment:

  • All medication that you are taking, in their original containers
