At the Diabetes Education Centre we provide education for patients with Diabetes Mellitus. The goal of this program is to enable patients to cope positively and gain independence in living with diabetes. Working collaboratively with our community partners to ensure patients have access to diabetes education in the care setting they require.

Our team consists of:

  • Certified Diabetes Educators, Registered Nurses, Registered Dietitians and Pharmacists
  • Endocrinologists
  • Chiropodists
  • Social Workers

 

Program Components include:

  • Individual assessments and follow ups
  • 2 day group education program
  • Gestational diabetes classes, with individual assessment, care and follow-up
  • Type 1 and Type 2 Diabetes prepregnancy planning
  • Type 1 and Type 2 Diabetes in pregnancy assessment, care and follow up
  • Insulin Initiation
  • Insulin Pump Initiation and follow up, Assistive Devices Approved Centre
  • Diabetes-related equipment instruction
  • Referral as required to chiropody or social work
