The GAIN Geriatric Clinic provides comprehensive assessment for frail seniors in the area. It is a collaboration between The Scarborough Hospital, Rouge Valley Centenary, Lakeridge Health and Peterborough Regional Health Centre, as well as the Community Care Access Centre and the Central East Local Health Integration Network.

The clinic will accept referrals for patients generally aged 75 and older, who:

  • live at home or in a retirement residence, or
  • have multiple complex medical, functional and psychosocial problems, or
  • are at risk for falls or have experienced falls, or
  • have recently experienced a functional or cognitive decline

Interprofessional Team

The GAIN Geriatric Clinic interprofessional team consists of:

  • Nurse Practitioners
  • Physiotherapist
  • Occupational Therapist
  • Social Worker
  • Pharmacist
  • Health Career Case Managers
  • Geriatrician or physician who specializes in geriatrics available for consultation when required

The team works collaboratively with the Geriatric Emergency Management (GEM) Nurse and with other professions within the ED to identify appropriate clients.

 

What to Expect

The GAIN Geriatric Clinic will see high risk seniors (aged 75+ years) from the Emergency Department and will accept community referrals from Family Physicians and Nurse Practitioners to assess, treat, and discharge home safely non-acute, stable seniors straight from the Emergency department avoiding a hospital admission. Additional home supports will be provided to ensure that the patient can remain at home until his/her next appointment.

Patients will be asked to bring any walking aids, eye glasses, hearing aids and ALL medications including over the counter products (all containers, bottles, eye drops, creams) to the clinic on the day of their appointment.

 

Referral Process

Referrals to the GAIN Geriatric Clinic must be signed by a Physician or Nurse Practitioner (NP/MD).

Completed referral forms can be faxed directly to The Scarborough Hospital’s clinic at 416-289-2961.

Patients will be contacted directly by the Clinic Clerk to book an appointment or will be given an appointment by the Emergency Department or the unit team when being discharged.

 

Communication

If necessary the clinic may need to contact family members, with patient’s consent, for more information. 

A summary of recommendations will be sent to the referring Primary Care Provider or Specialist following the clinic visit.

 

Clinic Operation

The Central East Local Health Integration Network (CE LHIN) is committed to the expansion of Specialized Geriatric Services.  

To be seen in the GAIN Geriatric  Clinic, clients must:

  • Be referred to the Clinic from a primary care provider, Emergency Department or upon discharge from the hospital
  • Must live in their home 

Ineligibility:

  • Long-Term Care Residents

 

Hours of Operation & Directions (as of May 24, 2011):

Monday to Thursday, 10:00 a.m. - 8:00 p.m.

Friday 9:00 a.m. - 5:00 p.m.

Saturday 9:00 a.m. - 5 p.m.
