The goal of the Heart Function Clinic is to manage your heart function care on an outpatient basis and to reduce ER visits and hospital admissions.  The multidisciplinary team, including a cardiologist and specially trained nurses will manage your heart health status through physical assessment and ongoing teachings about your medication, diet, and monitoring symptoms and conditions.  

A physician referral and booking request are preferred for appointment bookings.
