Intensive Case Management Service
The Intensive Case Management program offers long-term case management to individuals who have the capacity to live independently in the community with variable support. Up to twice weekly visits from our case management specialists are available.
Services Include:
- Accessing and maintaining independent housing
- Life skills and daily living skills teaching
- Goal planning
- Linkages to community services
- Supportive counselling
Who is Eligible?
A prospective resident must:
- Be at least 16 years of age
- Want to and have the capacity to live independently in the community with variable support
- Have needs not exceeding daily contact
- Be a consumer of the mental health system
- Be a Scarborough area resident and/or clients of The Scarborough Hospital (Birchmount or General Campus)
(Clients with a recent history of violence to others and/or uncontrolled violence are excluded)
