Acute Stroke Program/Short Term Stroke Rehabilitation

Stroke care is provided by interprofessional team that consists of physicians, nurses, pharmacy staff, social work, occupational and physiotherapy and speech and language pathology. Each patient’s care is coordinated by a nurse clinician.  TSH delivers stroke care within a regional model in affiliation with Sunnybrook Health Centre. Need for longer term rehabilitation is facilitated through the social worker.
