Patient Declaration of Values

As a patient of The Scarborough Hospital, I will receive the best healthcare at both the Birchmount and General campuses.


Knowing that each time I enter the hospital, I will be treated with courtesy, patience and respect by all staff members at all times.


I will receive high quality, safe, evidence-based medical care and services. In the event that these services are specialized elsewhere, I will be promptly referred and transitioned to that care provider.


I will be prioritized and informed promptly and my health condition will be diagnosed in an efficient manner.


My voice will be heard regardless of the language I speak or the cultural community with which I identify.


I will have access to timely and complete health information that includes options, so informed decisions can be made. I have a right to participate in decisions relating to my treatment and care.


I will feel confident knowing that all personal and medical information is kept confidential and handled in accordance with the Personal Health Information Protection Act, 2004.


I will be able to participate in the continuous improvement of the hospital and its services through community engagement and feedback.

Patient Rights and Responsibilities

As a patient of the Scarborough Hospital I have a responsibility to:


Treat others in the hospital, including my health care team, other patients and their families, visitors, volunteers, physicians and staff with respect and consideration at all times.
Recognize that the needs of other patients and families may sometimes be more urgent than my own.
Respect hospital rules for visitation and minimize noise for other patients.


Be an active partner in my care by participating with the health care team in my treatment and rehabilitation, providing accurate information, and questioning information and instructions that are not understood.


Use our precious health care resources wisely by arriving on time for all appointments and procedures, advising the hospital as early as possible if I am unable to keep an appointment and arranging for discharge, when appropriate. Have list of questions or requests ready to minimize multiple calls to hospital staff.


Ensure that my personal property, valuables and own medications are kept safe while I am in care. Send home any property that is not required.
Whenever possible, keep hospital facilities in good standard for other patients.


Share with hospital staff my satisfaction with the quality of care received and any observation of areas of potential improvement.
